Job Description

Lutheran Services Florida seeks a compassionate Case Manager to provide intensive support to children and families. In this role, you will manage a specialized caseload, conduct assessments, develop service plans, coordinate community resources, and advocate for client needs. You will monitor progress, maintain accurate documentation, and collaborate with counselors, educators, healthcare providers, and partner agencies. Ideal candidates bring strong crisis intervention, organizational, and communication skills, and a commitment to LSF's mission of strengthening families and protecting vulnerable children across Florida. Responsibilities Manage an intensive caseload of children and families requiring high-touch support Conduct biopsychosocial and risk/safety assessments to determine service needs Develop, implement, and regularly update individualized service and safety plans Coordinate services with internal programs and community partners, including healthcare, schools, and social service agencies Provide crisis intervention, advocacy, and ongoing support to clients Monitor client progress, outcomes, and compliance with plans and referrals Maintain timely, accurate, and confidential electronic case records and reports Participate in multidisciplinary team meetings, case staffing, and care coordination activities Educate clients and families on available resources, rights, and responsibilities Support LSF's mission and uphold professional and ethical standards in all client interactions Required Skills Case management Client assessment Service planning Crisis intervention Knowledge of social services systems Electronic case documentation Care coordination Community resource navigation Risk and safety assessment Report writing Time management Interdisciplinary collaboration
