The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of various social services, including workforce development, addiction counseling, and child welfare. The Case Manager will work collaboratively with clients to develop personalized plans that address their unique needs and challenges. Duties Conduct comprehensive assessments to identify client needs and strengths. Develop and implement individualized service plans in collaboration with clients. Provide crisis intervention and management to clients facing immediate challenges. Facilitate access to community resources, including housing assistance (e.g., Section 8), healthcare services (e.g., Medicare), and behavioral health support. Offer support in workforce development initiatives to help clients gain employment. Utilize motivational interviewing techniques to empower clients in their decision-making processes. Maintain accurate documentation of client interactions, progress, and outcomes. Collaborate with multidisciplinary teams to ensure holistic care for clients. Provide childcare support and resources for families as needed. Experience Candidates should possess a background in social work or a related field with experience in the following areas: Workforce development Addiction counseling Child welfare Crisis intervention Behavioral health Qualifications Strong interpersonal skills, empathy, and the ability to work effectively under pressure are essential. Experience with motivational interviewing and crisis management techniques is highly desirable. A commitment to advocating for the well-being of clients is paramount in this role.
