- Provides case management services through home visitations to pregnant women, parents of at-risk infants and women between pregnancies (interconception) to coordinate service delivery.
- Conducts assessments of client's total medical and social needs.
- Establishes rapport and develops relationships with families starting with program entry. Identifies, evaluates and assesses family strengths, resources, needs, and priorities. Facilitates planning and problem solving with participants and families.
- Provides information, education, and encouragement to inform and/or motivate families to change situations placing them at risk.
- Provides information and referrals to community resources.
- Develops and monitors the plan of care to assure that the concerns of families are addressed.
- Collaborates with other providers to assure continuity and coordination of care and advocates on behalf of the participant, including communicating to the providers, mental health counselors, networking agencies and community, the participant's strengths, needs, feelings and emotional and behavioral problems encountered during case management.
- Participates in staff development and training programs.
- Participates in team activities and meeting to coordinate, review, and recommend best practices.
- Accurately codes and documents services, education, and other pertinent information in participant's electronic record. Completed and submits all administrative documents.
- Perform other job-related services as required.
- The incumbent in this position may be required to use vehicles, owned, leased or operated by the state or county to conduct official state business. State or county vehicles should be used only for officially authorized duties and not for personal use, unless a specific exception applies under Florida law (F.S 287.17)
- Valid Florida Driver's license required.
- This position requires daily in - office or field work.
Required Knowledge, Skills, and Abilities:
Knowledge of theories and practices in counseling and social work. Knowledge of professional ethics relating to counseling and social work. Knowledge of interviewing techniques. Ability to provide counseling or social work services to others. Ability to provide casework services to clients. Ability to plan, organize and coordinate work assignments. Ability to actively listen to others. Ability to communicate effectively. Ability to establish and maintain effective working relationships with others. Knowledge of basic computer skills, including keyboarding, Windows and MS SuiteQualifications:
Minimum - A minimum of two years of verifiable professional experience in providing social work, health education, nursing or counseling services A minimum one year of verifiable professional experience conducting interviews or assessments of clients in a Health or Social Services setting Willing to physically come into the office daily and perform field work A valid FL driver's license or state identification card OR a valid driver's license from another state Willing to perform emergency duties before, during and/or beyond normal business hours and/or day in the event of an emergency, disaster, or threat of disaster man-made or natural Current authorization to work in the United States without employer sponsorship Preferred - Bachelor's degree from an accredited college or university or higher in social work, social sciences (psychology, human services, economics, biology, sociology and political sciences) or a health-related field as nursing, health education, health planning or health care administration Fluent in speaking Spanish and/ or Creole and communicating both verbally and written in proper English One year of verifiable professional experience in conducting home visits with clientsFlorida Department of Health Mission, Vision, and Values:
