Job Description

The Case Manager plays a vital role in supporting farmworker individuals and families through outreach, case management services, advocacy, and connections to essential community resources. This entry-level position is ideal for a compassionate, service-oriented professional who is bilingual in English and Spanish OR Haitian Creole and passionate about making a positive impact in the community. Key Responsibilities Conduct community outreach activities to identify, engage, and recruit eligible farmworkers and their families. Educate clients about available programs, community resources, and agency services. Complete client intakes, needs assessments, and required case management documentation accurately and efficiently. Maintain organized, confidential client records, case notes, and supporting documentation. Enter client information into agency databases and CMIS, ensuring timely completion of tracking reports and outcome measures. Meet program goals and performance objectives established by the supervisor. Provide transportation assistance to clients for medical appointments, social service visits, and other essential activities. Serve as an interpreter, translator, advocate, and support person during client appointments and interactions. Assist clients with completing applications, accessing benefits, understanding bills, and navigating community services. Refer clients to appropriate partner agencies and community-based resources. Facilitate support groups and deliver financial literacy workshops and educational sessions. Participate in staff meetings, community partner meetings, trainings, and professional development opportunities. Develop and maintain positive working relationships with community organizations and service providers. Assist with translating, preparing, and distributing outreach materials, flyers, and informational resources. Pick up donations and supplies as needed and provide donor receipts when applicable. Submit timesheets, reports, and required documentation accurately and on time. Work collaboratively as a member of a team while maintaining a professional, positive, and client-centered approach. Perform additional duties as assigned by the supervisor. Physical Requirements Ability to safely operate a motor vehicle and travel throughout the community to conduct outreach and provide services. Ability to bend, stretch, reach, lift, carry, and perform repetitive physical tasks as needed. Must be able to lift and carry up to 60 pounds. Qualifications and Requirements Fluency in English and Haitian Creole required. Strong communication, organizational, and interpersonal skills. Ability to maintain client confidentiality and handle sensitive information professionally. Valid driver's license and reliable transportation preferred. Commitment to serving diverse populations and promoting community well-being. Additional Information FWCC is a Drug-Free Workplace. Pre-employment drug screening is required.
