This role supports Veterans experiencing homelessness through individualized service planning, housing stabilization, benefits advocacy, and coordination of care across CRC's dual-track Low Demand (LD) and Clinical Treatment (CT) service model. Working under the supervision of the Lead Clinician, the Case Manager delivers Veteran-centered, trauma-informed case management designed to promote housing stability, recovery, increased income, and successful transitions to permanent housing. Essential Duties and Responsibilities Veteran Engagement & Service Planning
  • Participate in the admission and orientation process for newly enrolled Veterans.
  • Conduct intake assessments, housing readiness evaluations, and ongoing needs assessments.
  • Develop individualized service plans in collaboration with Veterans and interdisciplinary team members.
  • Engage Veterans using trauma-informed, strength-based, and recovery-oriented approaches.
  • Support Veterans in identifying personal goals related to housing, health, income, and wellness. Confidentiality and Privacy
  • Protect confidential Veteran information.
  • Maintain records in accordance with organizational policies and applicable privacy requirements.
  • Obtain appropriate authorization before communicating protected information when authorization is required.
  • Discuss resident information only with individuals who are authorized and have a legitimate need for the information.
  • Maintain appropriate privacy when conducting counseling, assessments, telephone conversations, and interdisciplinary consultations.
  • Understand and follow applicable requirements concerning behavioral health and substance-use treatment records. Case Management & Care Coordination
  • Provide intensive case management services including: o Housing stabilization o Benefits enrollment and income support o Employment readiness o Life skills development o Connection to medical, behavioral health, and substance use treatment o Identification documents o Transportation
  • Coordinate care with clinicians, residential specialists, and the Substance Use Disorder Counselor to support holistic recovery.
  • Facilitate referrals to VA Medical Centers and community-based providers.
  • Participate in case conferences and interdisciplinary team meetings. Housing Placement & Retention
  • Assist Veterans in identifying and securing permanent housing options.
  • Support Veterans with housing applications, documentation, landlord engagement, and move-in coordination.
  • Provide post-placement follow-up and housing retention support as required.
  • Coordinate with landlords, property managers, housing authorities, and housing programs when appropriately authorized.
  • Coordinate move-in needs and community support when available.
  • Document housing-search activities and outcomes. Documentation, HMIS & Reporting
  • Maintain timely and accurate participant records in HMIS and VA-required systems.
  • Document services, progress notes, and outcomes in accordance with CRC policies and VA GPD requirements.
  • Track key performance indicators including length of stay, income changes, housing placements, and successful exits.
  • Collaborate with Data and Compliance staff to ensure reporting accuracy and grant compliance. Compliance & Program Standards
  • Ensure Veteran eligibility documentation meets VA GPD standards.
  • Adhere to all program policies related to confidentiality, grievance procedures, safety protocols, and participant rights.
  • Support monitoring visits and audits by maintaining organized case files.
  • Participate in quality improvement activities and staff training. Team Collaboration
  • Work closely with Residential Specialists to support daily Veteran needs and engagement.
  • Coordinate with Clinicians to align service plans with behavioral health goals.
  • Communicate regularly with the Project Director regarding Veteran progress, barriers, and service needs.
  • Contribute to a supportive, mission-driven team culture.
Physical and Environmental Requirements The employee must be able to perform the essential functions of the position with or without reasonable accommodation.
The position may involve:
  • Sitting for extended periods while completing documentation.
  • Walking throughout the residential facility.
  • Lifting over 25 pounds
  • Responding promptly to behavioral health emergencies.
  • Working in environments where residents may experience emotional distress, behavioral escalation, or psychiatric symptoms.
  • Operating computers, telephones, and standard office equipment.
  • Occasional schedule adjustments in response to program or crisis needs.
  • Travel to program sites in Miami-Dade and Broward. Required Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, or related field.
  • Minimum two (2) years of experience providing case management or supportive services to vulnerable populations.
  • Experience working with individuals experiencing homelessness, behavioral health challenges, or substance use disorders preferred.
  • Strong documentation and organizational skills.
  • Ability to engage diverse populations with empathy and professionalism.
  • Basic computer proficiency and familiarity with HMIS or electronic case management systems. Preferred Qualifications
  • Master's degree (MSW or related field).
  • Experience serving Veterans.
  • Knowledge of housing stabilization or Housing First/Low Demand models.
  • Experience in residential or clinical settings.
