The Center for Family and Child Enrichment (CFCE) is dedicated to help children and their families by providing the right services and solutions based on individual needs. CFCE is constantly evolving to better support our community. The Case Manager provides comprehensive support to individuals residing in the group home by assessing their needs, developing individualized service and care plans, and coordinating appropriate supports and community-based services. This role works closely with residents, staff, families, and external providers to ensure continuity of care, promote stability, and address both immediate and long-term needs. The Residential Case Manager also serves as an advocate for residents, helping them access essential resources, achieve personal goals, and improve overall well-being, independence, and quality of life. Functions Include:
Conduct intake assessments and develop individualized care plans. Coordinate services with healthcare providers, social workers, and community resources. Provide direct support, counseling, and life skills training. Advocate for residents' rights and access to education, employment, and housing. Maintain accurate case documentation and monitor resident progress. Address mental health and substance use concerns; provide crisis intervention. Participate in team meetings and ensure compliance with policies and regulations. Perform other duties as assigned. Minimum Education Bachelor's degree in Social Work, Psychology, Human Services, or related field. Experience in residential case management or similar role. Proficiency in case management software and Microsoft Office. CPR and First Aid certification (may be required). Licensing or certification (e.g., Certified Case Manager) as applicable. Skills Needed Strong documentation, organizational, and time management skills. Knowledge of community resources and support services. Excellent communication, empathy, and conflict resolution skills. Ability to collaborate, adapt, and work with diverse populations. CFCE is a Drug Free Workplace and an Equal Opportunity Employer.
