Job DescriptionJob DescriptionYour MissionAs a Case Manager, you'll keep recovery moving—connecting clients to the right supports at the right time across the continuum of care. This role exists to remove barriers, strengthen engagement, and help individuals build a workable plan for lasting change.

Key responsibilities
  • Complete biopsychosocial and case management assessments; co-create individualized service plans with the client and team
  • Track progress toward treatment and recovery goals and maintain consistent client contact
  • Coordinate referrals for medical/psychiatric care, MAT, housing, employment, financial/legal supports, transportation, and community recovery resources
  • Advocate for access to needed services and assist with applications (e.g., Medicaid, SNAP, IDs
  • Document all activity in electronic health records; follow HIPAA and 42 CFR Part 2; contribute in treatment team meetings and respond to crises appropriatelyRequired knowledge/skills
