10/05/2026 3:02 PM Case Manager (Senior Services) Full-Time $22.68/Hour Responsible social work in determining client services and maintaining accurate records. Work is performed under the general supervision of the Program Manager or designee.
Responsibilities:
Visits clients in their homes for completion of assessment and client directed plan that determines service needs and determines which providers will fulfill those needs. Works with client, family and caregiver(s) to develop a care plan utilizing subcontracted providers and community resources to fulfill unmet needs. OCMC will sign off on care plan after discussion and review. Assists in coordinating and arranging services from initial order and follow up, annual and semi-annual assessments through termination from program(s). Maintains a case load of approximately seventy-five seniors for in-home services in addition to completing annual assessments for congregate dining clients and home delivered meal clients. Resolves conflicts and service provision problems as they arise with support from OCMC and Program Manager. Completes assessments and reports promptly and accurately. Responds to Adult Protective Services (APS) referrals immediately. Responsive to emergency assignments (i.e. shopping, buying medicine) for at-risk clients and seniors when needed. Requirements:
Bachelor's degree in social work or related field is required. Two (2) years' experience in Outreach, other social services, or working with elderly clients. Valid Florida Driver's License Level II Background Screening (A comparable amount of training, education or experience can be substituted for the minimum qualifications.)
