Position Summary:

- We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team.
- Required Qualifications
- Bi-lingual, English, and Spanish.
- Must reside in zip code 33172
- Willing and able to travel 75% of their time to meet members face to face in
- Fontainebleau Miami, FL. 33172
- Computer proficiency in Microsoft Word, Excel, and Outlook required.
- 1 + year of experience in behavioral health or a long-term care setting.
- Preferred Qualifications
- Strongly prefers Field Case management and discharge planning experience.
- Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment.
- Effective communication skills, both verbal and written.
- Managed Care experience.
- Education
- Bachelor's degree required, preferably in social work or related field required
