The Lead Case Manager oversees intake, assessments, service planning, and ongoing case management to promote housing stability, employment, and self-sufficiency. Responsibilities include supervising case managers, coordinating care with internal and community partners, tracking outcomes, and ensuring trauma-informed, client-centered services aligned with the organization's faith-based mission, values, and program standards. Responsibilities Lead and supervise case managers within residential programs Conduct and oversee client intake, assessment, and service planning Provide intensive case management to support housing and economic stability Coordinate services with internal teams and community partners Monitor client progress and adjust service plans as needed Ensure trauma-informed, strengths-based, and client-centered practices Maintain accurate documentation and reports for outcomes and compliance Support program development, quality improvement, and best practices Provide crisis intervention and advocacy for residents Model and uphold the organization's faith-based mission and values Required Skills Case management Supervision and staff leadership Client assessment and service planning Motivational interviewing Trauma-informed care practices Crisis intervention Knowledge of homeless and housing resources Data entry and case documentation Program compliance and reporting Interdisciplinary team coordination
