MY SISTER'S PLACE CASE MANAGER JOB DESCRIPTION ABOUT MSP

My Sister's Place (MSP) is a non-profit Christian-based organization that opens its doors to provide safe shelter, nourishing food, a nurturing environment and resources for women and their children who are experiencing homelessness. MSP. THE JOB

The Case Manager is responsible for performing tasks to achieve the best outcomes for MSP shelter residents and transitional housing tenants ("Clients") towards housing stability and independent living. These duties involve working directly with the clients and representatives of human service agencies in the community to achieve healing, growth and housing stability. The position reports directly to the Shelter Manager.

RESPONSIBILITIES

The Case Manager is responsible for providing client case management for single women as well as mothers and their children as long as they are who are residing at MSP, whether in the shelter or transitional housing units. Under the direction of the Shelter Manager and in partnership with night staff and volunteers, the Case Manager is responsible for ensuring that client needs are well understood and mutually agreed upon with clients and that clients are well supported as they work toward their goals. Duties of the Case Manager include, but are not limited to:

•Based upon MSP's core values, ensuring MSP is always a welcoming place to all, and that residents, tenants, donors, co-workers, guests and leadership are provided excellent service Providing individualized case management for up to 20 individuals and/or families in the MSP program Completing self-sufficiency assessments and supporting MSP clients as they develop and implement their Self-Sufficiency Achievement Plan Meeting a minimum of every two weeks with each household to assess progress and to provide guidance, feedback, encouragement Assisting clients with developing and implementing strategies that support them in achieving their self-sufficiency and personal growth goals such as job searches and readiness, education goals, childcare assistance, financial health, relational health, growing relationship with Jesus Christ, etc. Staying current on community resources and contacts, advocating for clients as appropriate for assisting with their progress towards independence Arranging meetings between clients and supporting agencies as needed Monitoring, evaluating, and documenting client program progress as required Achieving an effective handoff for continuing support at the end of their time at MSP Participating in case review meetings, team meetings, on-going training, and coaching sessions for career development, to stay informed, and to ensure fidelity to MSP's mission Identifying opportunities in which the lived experience and voice of clients served can improve program operations Ensuring timely communication with the Shelter Manager, sharing all information necessary for MSP leadership to make informed decisions including any significant incidents involving shelter residents Other duties as assigned

QUALIFICATIONS
REQUIRED:

Bachelor's Degree in a human services field from an accredited college or university and two years' of case management and support experience; or Associates Degree from an accredited community or junior college and four years' of case management and support experience.

PREFFERED

Experience working in a non-profit organization or faith-based organization; HUD housing knowledge.

SKILLS:
REQUIRED

Proficient in Microsoft Office Applications (Word, Excel, Outlook) Able to learn web-based tracking tools Ability to work effectively with other professionals and non-professionals Highly sensitive to the need's single women and families Highly self-motivated and able to work independently when required Excellent interpersonal skills, written/verbal communication and presentation skills Must be well organized and self-directed

$36,000.00 - $40,000.00 per year
