Compensation:
$23 - $25 hourly
Responsibilities:
Assist Long-Term Care Case Manager with Medicaid Matters. Perform long-term care consultations with potential clients to determine Medicaid Eligibility. Collaborate with team members to develop comprehensive estate and long-term care plans. Process Nursing Home Medicaid Applications. Coordinate with healthcare professionals to offer holistic client support; assist clients as needed with nursing home, assisted living, or Independent placement, or in-home caregivers. Visit Care Facilities to deliver brochures and educate on our services.
Qualifications:
The ideal candidate has 3-5 years of experience with Nursing Home Billing, DFCS Medicaid Case Management, or as a Medical Social Worker. Ability to conduct thorough consultations to assess Medicaid eligibility. Proven track record of processing Medicaid applications efficiently and accurately. Strong collaboration skills to work effectively with team members and healthcare professionals. Excellent communication skills for educating care facilities about our services. Ability to work with attorneys to develop comprehensive estate and long-term care plans tailored to client needs. Proficiency in coordinating client support for nursing home, assisted living, or in-home care placements.
