Position Summary:

The Youth Homeless Demonstration Program ( YHDP ) Case Manager provides intensive, individualized case management to youth and young adults (ages 18-24) with dependents who are experiencing or at risk of homelessness. The Case Manager partners with participants to develop individualized housing stability plans that promote long-term self-sufficiency through stable housing, education, employment, life skills, and community connections. Using a trauma-informed, strengths-based, and youth-centered approach, the Case Manager assists participants in overcoming barriers while supporting their journey toward independence.
