Job Description

Innovative Health Care Concepts (IHCC) seeks a Case Manager and/or CBRS worker. These are professional-level services that work with children, families and adults with mental health illnesses. Applicant must have at least a bachelor's degree in a
HUMAN SERVICE FIELD
. Experience working with individuals with mental illness is preferred but not required. Bilingual (English/Spanish) applicants are preferred but not required. The wage will vary based on the service provided. Pay is by billable hour. The position is part-time to start but can grow to a full-time caseload.
Case Management job responsibilities include:
  • Providing Case Management services in a culturally competent manner . Participating on a multidisciplinary treatment team to identify and meet individual and family needs
  • Carrying out case management activities in the home and community
  • Consulting and collaboration with other mental health professionals
  • Connecting clients to identify community resources
  • Creating the Case Management Plan with the client
  • Completion of 90
  • 120
  • and 270-Day progress reviews CBRS job responsibilities include:
  • Carrying out individualized CBRS therapy in a culturally competent manner
  • Teaching practical life skills such as socialization, hygiene, problem-solving, and budgeting
  • CBRS services are provided in both the home and community
  • Collaborating and creating the CBRS treatment plan with the client, family, and therapist
  • Participating on treatment teams to meet individual and family goals
  • Provides direct support and practical skill training to children and/or adults
  • Completion of 90
  • 120
  • and 270-Day progress reviews
  • Timely and accurate documentation of all services provided.

PAY DEPENDS ON EXPERIENCE IHCC
