Our Case Management Services help individuals create fulfilling lives with maximum independence by providing advocacy, support, and linkage to community resources.
Job Scope:
Work in a court-based role, primarily working with individuals referred through the court system who have persistent or recurrent mental illness. This position supports clients in achieving stability and independence by coordinating access to housing, food, transportation, and entitlements, while providing counseling, life skills training, and strong advocacy. The Specialty Courts Case Manager collaborates closely with court personnel, oversees court referrals, and participates in court staffing to ensure alignment between treatment and legal requirements. Responsibilities also include completing clinical assessments and developing individualized, strength-based, recovery-oriented treatment plans, responding to intake and emergency needs, and maintaining timely, accurate documentation to support continuity of care. Primary Responsibilities Assist adults with persistent or recurrent mental illness live independently in the community via maintaining housing, food & transportation resources, applying for entitlements, providing counseling and life skill training and advocating for the client's needs Complete Mental Health Assessments, DLA and LOCUS, and develop goal-oriented ITPs that are strength-based and recovery oriented on all assigned clients as required Oversee court referrals and attend court staffing as appropriate Provide assistance with intake/assessment as needed, as well as outreach in emergency situations Provide accurate, quality documentation within a maximum of 48 hours after service delivery Performance Requirements Demonstrate knowledge of documentation standards and capacity to meet documentation expectations. Ability to get clients to participate in individual sessions. Ability to assess, diagnose, interpret and integrate both verbal and non-verbal client input during session. Ability to clinically "listen" during session. Ability to establish rapport with clients and interact constructively with others in the workplace. Ability to focus client on individualized treatment plan and goals. Give information in a clear and understandable fashion and respond constructively to appropriate client questions and concerns. Education and Experience Bachelor's degree in human service field or equivalent experience One-year clinical experience Must have a valid Illinois driver's license, auto insurance, automobile, and a willingness to transport clients as necessary Schedule Monday•Friday 9am•5:30pm (40 hours a week).
