General Responsibilties Provide casework and follow-up services to substantiated victims, vulnerable adults, and verified abusers to prevent further abuse, neglect, or exploitation. Receive and document reports of abuse, neglect, self-neglect or exploitation of older adults and persons with disabilities. Conduct case consultations with the Adult Protective Services Supervisor for care plan development and case closure. Provide follow-up services with face-to-face and phone contacts every three months for up to twelve months, longer when permitted Document all activities on required forms, adhering to APS program policies and time frames. Make referrals for appropriate follow-up services such as case management or community-based services. Prioritize and respond to client needs within specified time frames. Work effectively with co-workers and other community agencies. Attend staff meetings, in-services, conferences, and workshops as recommended. Be an active member of the APS Multi-disciplinary Team. Document all activities on required forms, adhering to APS program policies and time frames. Other duties as assigned within the guidelines of this position.

Passage of complete background clearance and other required testing. Valid driver's license, reliable transportation, and proof of liability insurance.
