As a Community Based Social Worker, you will play a vital role in facilitating and coordinating services for those experiencing complex challenges. Your main objective will be to ensure that they receive the support and resources necessary to achieve their goals and improve their overall well-being. Our Wraparound Program is an intensive, strengths-based, and family-driven process that supports families staying together. The program is a planning process that follows a series of steps to help children and their families realize their goals.
Key Responsibilities:
Service Delivery:
Implement a family centered, strengths based, trauma informed plan that addresses the needs of the child and family while using their natural supports and strengths to reach goals identified by the child and family team. Duties may include developing a child and family team, attending meetings, assisting in identifying strengths/needs and establishing goals, and developing strategies to overcome any barriers. Documentation Ensure all documentation and reports are accurate and completed in a timely manner. Duties may include assessments, notes, progress and closing reports, and compliance documents.
Community Relations:
Assist in representing the agency and advocating for needed services. Ensure collaboration and strong communication with community partners and referral sources.
Qualifications:
Bachelor's Degree in a Human Services field and a valid driver's license required. Previous case management experience in social services and/or child welfare and experience working with community-based programs preferred.
Skills:
Must be organized, able to multi-task, have excellent written and verbal communication skills, basic computer skills, and detail oriented. Must be able to interact with, and relate to, diverse groups of people.
Miscellaneous:
Must pass all pre-employment drug screens and background clearances. Must maintain the ability to transport clients per DCFS standards. Must be willing to travel and work flexible hours to meet the needs of the program. Required to work in clients' homes and community settings.
