(Recover, Education, Support and re-Entry Team) is seeking a dedicated and compassionate Case Manager to provide comprehensive case management services for the adult reentry participants as they navigate the reentry process. This position will provide case management and transitional management to persons currently incarcerated in the McLean County Detention Facility. Work closely with other

staff, medical services, as well as other community partners to monitor progress, and coordinate referral support. This position will also work with external partners (such as literacy, behavioral health and housing providers, benefits, etc) to ensure that services are coordinated, and participants are able to fully engage in a holistic reentry process. Work is performed according to state and county jail standards, detention facility and county policies and procedures, and administrative orders of the Sheriff. Work is reviewed by the Director of

for accuracy of records and proper classification of inmates. Essential Duties and Responsibilities (May not include all duties performed) Conduct thorough assessments of residents' needs and strengths to develop personalized service plans. Assist in crisis intervention and support to residents in distress, ensuring theirs and staff's immediate safety and well-being. Collaborate with healthcare providers, social services, and community organizations, courts, and probation offices to coordinate care and resources. Facilitate access to community services such as Medicare, behavioral health programs, addiction counseling, and housing assistance. Maintain accurate documentation of resident interactions, progress notes, and case files in compliance with organizational policies. Provide program classes and support to clients in developing life skills and achieving personal goals. Develop and maintain positive relationships with others, including training partners, correctional staff, program participants, probation/parole officers and other collaborating partner staff. efforts to increase returning citizens' access to more individualized and higher quality services to address health, addiction, employment, education, economic mobility, and/or career pathways. Other duties as assigned. Knowledge, Skills and Abilities Strong written and verbal communication skills Extremely proficient with computers Ability to multi-task and manage competing priorities Familiarity with local resources and social service agencies. Experience resolving routine issues utilizing external resources and escalating internally when appropriate Ability to communicate to others Minimum Qualifications A Bachelor's degree in counseling, rehabilitation counseling, social work, psychology or related field with a degree from an accredited college or university. Preferably with major course work in criminal justice, psychology, sociology, social work, or related social sciences, and experience in the criminal justice field, preferably dealing with incarcerated persons or any equivalent combination of education and experience that would provide the above noted knowledge, skills and abilities. Work Environment Work is performed indoors and outdoors during surveying and construction and maintenance activities for extended periods of time under a wide variety of weather conditions. Work may expose employees to inclement weather, poisonous plants, fumes, and dust. Work requires the use of safety clothing and equipment. May involve dealing with distraught and angry residents.
