Job Description

Position Summary The Self‑Directed Assistant (SDA) Case Manager supports individuals with intellectual and developmental disabilities (I/DD) who participate in the Illinois Medicaid Waiver Home‑Based Services Program. The role helps individuals and families manage self‑directed services, understand waiver rules, and coordinate supports while promoting independence and informed choice. Essential Duties and Responsibilities
  • Assist with person‑centered planning, goal setting, and service coordination.
  • Help individuals and families understand and manage their annual HBS budget.
  • Support employer‑related tasks such as recruiting, onboarding, and supervising Personal Support Workers.
  • Ensure documentation, spending, and service use comply with DHS/DDD requirements.
  • Maintain communication with the individual, family, ISC agency, and service providers.
  • Connect individuals to community resources and advocate for their needs.
  • May be required to temporarily fill in for scheduled staff when unexpected absences occur.
  • Promote independence by encouraging skill-building in daily living activities
  • Maintain accurate documentation of services provided, including time, tasks completed, and any notable observations
  • Communicate effectively with supervisors, team members, and families/guardians as appropriate
  • Ensure the health, safety, and well-being of individuals at all times
  • Follow all applicable Illinois regulations, agency policies, and confidentiality
  • Flexibility with scheduling per the individuals standards is required as is punctuality and reliable attendance. Qualifications
  • Associates required
  • Bachelor's degree preferred
  • Must have or be willing to complete DSP certification within state-required timelines (training provided).
  • Previous experience in caregiving, human services, or a related field preferred
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • Ability to pass background checks in accordance with Illinois requirements
  • CPR/First Aid certification (or willingness to obtain)
  • Ability to work independently and manage time effectively between multiple client visits.
  • Ability to lift up to 25 lbs. and assist individuals with mobility or transfers as needed.
  • Must pass all DHS/IDPH-required background checks, physical, and drug screening.
  • Valid driver's license and reliable transportation strongly preferred.
Work Environment The SDA Case Manager will work in a professional office setting located at the agency's main facility. While the position is primarily office‑based, the SDA Case Manager will also travel to participants' homes or community locations as needed to complete assessments, provide support, attend team meetings, or respond to service needs. The work schedule is generally Monday through Friday during standard business hours, though occasional flexibility (nights and weekends) may be required to meet participant needs.
