Case Manager Position Summary:

BEDS Plus provides services to remediate homelessness in southwest suburban Cook County. We are a 35 year old organization with roots in La Grange. Our mission is to help people obtain and retain housing. We do this by working with those is need to build and work on plans which remove the barriers to accessing housing. Working closely with our clients, BEDS case managers are often successful in providing the assistance needed to access housing. The case manager will oversee and deliver case management services to to those assigned to them. Currently, we have three case manager openings in various lines of work, including domestic violence, permanent supportive housing and interim housing.

Duties:

• Completion of Continuum of Care (COC) and departmental training (1st 60 days).
• Provides case management for up to 25 people
• Manages client pathways, including but not limited to proper assessment, pathway identification and timely qualifications for each client to become stably housed.
• Responsible for accurate & timely reporting including incident management.
• Responsible for client care represented by the completion of timely and accurate intake, assessment, and individualized service plans.
• Responsible for connecting clients with basic needs such as clothing, food, access to health/behavior health services, access to purposeful transportation and other needs outlined in the individualized service plans.
• Work cooperatively with client, staff and operations manager to ensure minimum room habitability standards are met.
• Responsible for maintaining professional and accessible office space(s) including an inventory of basic needs.
• Responsible for enforcing rules and protocols with participants. Support the development of program standards.
• Responsible for HMIS entry including intake, audit of staff entry, nights of shelter and program entry & exit.
• Support the coordination of day-to-day operations including but not limited to room turnover, food coordination and staff scheduling.
• Maintain a positive relationship with staff in complimentary programs to ensure successful client entry and exit from triage program.
• Work with Director to determine proper exit.
• Maintenance of Outlook calendar with client appointments and agency meetings as required agency-wide.

Data responsibilities:

• Responsible for integrity and confidentiality of client files and data quality in HMIS.
• Responsible for file preparation and follow-up as program audit occurs.
• Ensure complete and accurate reporting of work, including time accounting and client tracker/regular client reports.
• Ensure monthly submission of required performance tracking.
• Adhere to relevant COC, HUD, County or State program manual/guidelines/regulations.

Physical Requirements:

This position requires sitting, standing, driving and lifting as much as 50 lbs. _______________________________________________________________________________________________

Education and Qualifications:

Bachelor's of Social Work or related Bachelor's degree is required plus at least 3 years of case management experience working with people with mental illnesses, substance abuse disorders or homeless population. Reports to :

Director of Emergency Services Hours and Location :

Full-time. Static schedule that includes at least 1 weekend day and some evenings.
