Job Description
Overview We are seeking a dedicated and compassionate Social Service Coordinator to join our team. This vital role involves coordinating and delivering comprehensive social services to diverse populations, including seniors, individuals with disabilities, and those experiencing behavioral health challenges. The ideal candidate will possess a strong background in social work, crisis intervention, and resource identification, with a passion for empowering clients through effective case management and community engagement. Responsibilities Assess clients' needs through thorough interviews and evaluations, developing personalized care plans aligned with their goals. Experience working with residents who are elderly, ill, disabled, or emotionally distressed. Experience coordinating admissions and discharge planning in a healthcare or long-term care facility. Experience collaborating with interdisciplinary care teams and external community health or social service agencies. Experience providing counseling related to behavioral or mood issues for residents and families. Experience ensuring compliance with healthcare-related regulatory requirements (e.g., PASARR, Medicare/Medicaid admissions rules) Conduct client communication with empathy and professionalism, ensuring clarity and respect in all interactions. Requirements Bachelor's degree in social work, psychology, sociology, or a related field; relevant experience may substitute for formal education. Proven experience working with seniors, individuals with disabilities, or geriatric patients within social services or healthcare settings. Knowledge of Medicaid, Medicare, helpful. Strong case management skills with the ability to coordinate multiple resources effectively. Excellent communication skills Ability to handle sensitive situations with discretion while maintaining a client-focused approach in high-pressure environments.
