Case Managers:

Help individuals discover their strengths and set personal goals. Lead person-centered planning (PCSP). Navigate services like Day and Residential programs, Medicaid/MCOs, SSI/SSDI, SNAP, VR, mental health & medical support. Advocate for choice, rights, and independence. Coordinate services and build partnerships in the community. Balance meaningful relationships with important documentation and case management tasks. You'll thrive if you are: Organized and detail oriented. A strong communicator and advocate. Passionate about person-centered support. Skilled at navigating resources and helping people with problem-solving.

Requirements:

Bachelor's degree and 6+ months experience with IDD/Intellectual and Developmental Disabilities OR equivalent experience in the field. Valid driver's license and good driving record.
