Essential Duties include:

Serve as the client advocate for all persons assigned to receiving case management or related services in a manner that assures the protection of rights, the maintenance of confidentiality, and ongoing client satisfaction in all issues relating to service provision. Assure compliance of all client files/documentation with applicable licensing and accreditation requirements. Provide and share information about services and resources available, provide referrals, and link individuals to the support and services they need. Keep abreast of current developments in community-based service delivery. Monitor, implement, and successfully manage state/federal funding program procedures to ensure program compliance and maximize allowable billing. Coordinate, develop, and monitor person-centered plans in a manner that helps establish natural supports and community inclusion. Complete or coordinate the completion of assessments as determined necessary by support needs or person-centered plans. Communicate effectively with individuals served, their families, designated representatives, and support and service providers relative to the service plan and related support needs.

Education and Experience Required:

Bachelor's degree in education, psychology, or other human services-related field preferred and a minimum of 6 months of full time experience in human services. Relevant full-time employment experience in developmental disabilities services may substitute for any portion of a bachelor's degree at the rate of six (6) months of full-time employment experience for each semester. Valid Kansas drivers license.
