Office, Client Homes, Schools & Community-Based Settings Position Summary FamilyCare Counseling Solutions is seeking an organized, compassionate, dependable, and highly motivated Intake Case Manager to join our behavioral health team. This position combines behavioral health intake and admissions coordination with community-based Targeted Case Management (TCM) . The Intake Case Manager helps new clients successfully move through the continuum of: Referral → Intake → Assessment → Admission → Treatment Engagement → Ongoing Care Coordination In addition to coordinating new referrals and admissions, this position maintains a manageable TCM caseload of up to approximately 25 individuals and provides ongoing care coordination consistent with Kentucky requirements and FamilyCare's TCM program standards, and will transfer cases after 30-90 days to the ongoing case management team. This is an hourly W-2 employee position and is not a contract or PRN role. The position includes established work expectations while allowing appropriate flexibility for community visits, client needs, treatment-team meetings, and admission activities. Schedule & Work Expectations Because this position combines admissions responsibilities with community-based case management, the Intake Case Manager must be able to independently organize their workday while maintaining consistent availability and attendance.
The employee will:
Maintain an established hourly W-2 work schedule . Accurately clock in and out using FamilyCare's timekeeping system. Maintain dependable and consistent attendance. Coordinate office, home, school, and community-based appointments throughout the workweek. Adjust daily activities based on admissions, client needs, treatment-team responsibilities, and operational priorities. Complete required monthly TCM contacts within established timelines. Complete at least one home/community-based visit per month for each assigned client , consistent with program expectations and client needs. Attend required staff meetings, supervision, treatment-team meetings, and training. Maintain appropriate communication with the Clinical Supervisor regarding schedule changes, field activities, client concerns, and barriers to completing assigned responsibilities. Participate in on-call responsibilities when assigned based on program needs. This is not exclusively an office or remote position. Regular travel within the service area and face-to-face interaction with clients and families are essential components of the role. You will work closely with therapists, the Clinical Supervisor, administrative staff, families, schools, healthcare providers, DCBS/CHFS, and community organizations to help clients successfully access and remain engaged in behavioral health treatment.
