Job Description
Job Overview We are seeking a dedicated and compassionate Case Manager to join our team in our men's residential location. The ideal candidate will play a crucial role in coordinating client care, assisting them in developing life skills, and supporting clients through their treatment journey. This position requires a strong understanding of court systems, assisting with appointments and connecting them with local and community resources in preparation for life beyond treatment. The Case Manager will also be responsible for maintaining compliance with HIPAA regulations and advocating for clients' needs. Responsibilities Assess clients' needs and develop individualized case plans that align with their treatment goals. Coordinate with multidisciplinary teams to facilitate effective transitions through treatment. Serve as a liaison between clients, families, court systems and local resources to ensure comprehensive client service. Monitor progress and adjust plans as necessary to optimize outcomes. Provide education and resources to clients regarding their treatment options, including behavioral health services when appropriate. Maintain accurate documentation of case management activities in compliance with HIPAA regulations. Participate in case reviews and contribute to quality improvement initiatives within the organization. Qualifications Bachelor's degree; relevant certifications in case management preferred. Experience in managed care settings, substance abuse treatment, or outpatient services is highly desirable. Strong knowledge of discharge planning processes and behavioral health principles. Excellent communication skills with the ability to build rapport with clients and families. Proficient in case management software and electronic health records (EHR). Ability to work collaboratively within a team while demonstrating strong organizational skills. Commitment to providing high-quality client care while adhering to ethical standards.
