Job Description

Gulf Coast Social Services is seeking a dedicated and compassionate Case Manager to join our team at the South Central Louisiana Human Services Authority (SCLHSA) Terrebonne Parish Behavioral Health Clinic. The Case Manager provides clinical case management activities across the continuum of care from assessing and planning to implementing, coordinating, monitoring, and evaluating individuals and families in assigned clinic locations. Candidates should possess good verbal, written, and interpersonal skills, the ability to represent the Agency in a highly professional manner, and to maintain the highest level of confidentiality. As well as good judgment, initiative, dependability, sound decision-making skills, the ability to handle multiple responsibilities, strong organizational and time management skills, and the ability to prioritize work and meet multiple deadlines
Essential Job Functions:
Assist with triage and screening of all walk-in clients. Ensure confirmation of assessment and treatment appointments. Identify and process/ mail letters to clients who miss their appointments. Complete and fax Client status reports to designated personnel. Prepare and send referrals Administer urine screenings and breath alcohol tests, as needed Prepare/disseminate Interim Packets Provide Crisis Intervention Services Provide back-up support for the Administrative Coordinator, including but not limited to answering phones, screenings, checking in/out clients, and filing charts.
Requirements:
Master's Degree (preferred) in counseling, human services, or a related field Bachelor's Degree (required) in counseling, human services, or a related field Must have the ability to pass a criminal background check - if selected for the position Must have the ability to pass pre-employment drug screening - if selected for the position Valid Louisiana driver's license, proof of auto liability insurance, and access to a reliable vehicle in a safer operating condition with a current inspection sticker Gulf Coast Social Services is an
