A Case Manager coordinates delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs.
Overview The Case Manager/Case Coordinator oversees the Direct Service Worker(s) and is directly responsible for coordinating all healthcare case aspects related to the staff, treatment, and programs as well as ensuring quality, professional services with the highest level of respect to ensure the dignity of our clients.
Essential Duties:
Abiding by and enforcing all agency Policies and Procedures May supervise Field Supervisors and Direct Support Workers (DSWs) Ensuring DSWs work all scheduled hours and documenting non-worked hours Updating clients' Home and Office binders as needed Maintaining all clients' Office and Home binders to ensure compliance. Purging clients' binders quarterly, and purging documentation in clients' charts as needed Attend all required training and meetings Ensuring DSWs receive required training according, and train DSWs on progress notes, coaching and editing as needed Coordinating with HR on all staff termination and clients' discharges Perform detailed weekly reporting, administrative filing and documentation Conduct unannounced supervision visits to DSW's 60 days of hire, and annually thereafter to evaluate staff Conduct scheduled and unscheduled client check-ins for wellness and safety Ensure agreements are sent to provider services as needed Answer phones, return voice messages within 24 hours, transfer calls as needed, and take detailed messages when necessary Effectively communicate with clients and staff regarding complaints, concerns and needs with regards to services. Document and report all concerns the Administrator Attend CPOC meetings and assure services are delivered as per CPOC and/or ISP Provide on-call duty rotation Maintain workplace safety, hygiene and cleanliness at all times Print Prior Authorization renewals monthly Must always be available in person or by telecommunication to assure smooth and continuous delivery of services Effectively communicate with CEO, administrators, human resources, staff, peers and support partners Ensure that services meet all of the state and federal regulations Proficient use of Microsoft Excel to provide daily data entry Monitor and document service delivery for case load Review staff timesheets and progress notes weekly for accuracy Act as a liaison between Loving Hearts and families Other duties as assigned by the
Administrator Qualifications:
Bachelor's degree in human services (i.e. Nursing, Social Work, Psychology, Gerontology, Rehabilitation Counseling and Health Care Administration) Bachelor's degree may be substituted for at least 2 years of experience in working with people who have disabilities and/or people who are elderly Must be a resident of Louisiana Knowledge of the AAHSD and OAAS funded services Current Driver's license Current vehicle insurance Three valid references (Employment is preferred; personal will be considered) Criminal background check clearance Driving history clearance Loving Hearts of LA is an Equal Opportunity Employer.
