Counselor/Case Manager 3.5 3.5 out of 5 stars 270 Communication Way, Hyannis, MA 02601 $25.50 - $26.50 an hour - Full-time Old Colony YMCA Barnstable Community Justice Support Center 124 reviews $25.50 - $26.50 an hour - Full-time There are 18 Office of Community Corrections Community Justice Support Centers across the state including 10 operated by the Old Colony YMCA. Clients attend CJSC to participate in programs designed to improve their lives and avoid further criminal justice involvement. Each CJSC offers about 15 different programs, including cognitive behavioral treatment for Substance Use Disorder (SUD) and to improve decision making, employment counseling, and Adult Basic Education, GED/HiSET, and post-secondary preparation. The Counselor/Case Manager is responsible for the overall implementation, assessment, and administration of quality clinical and casework services at the Barnstable Community Justice Support Center, a community based facility serving adults (male and female) involved with the justice system who are experiencing addiction issues.
Typical Job Responsibilities include:
Ensure that all casework services are consistent with the operating policies of the Office of Community Corrections (OCC) and other regulatory agencies. Ensure that participant documents such as a copy of all intake materials are placed in participant files including those provided by supervising agencies (e.g., Probation, Parole, and Sheriff's Department). Complete intake assessments utilizing assessment tools (e.g. LS/RNR, TCUDS, etc.); assess risks/responsivity needs; and develop treatment plans for each assigned participant. Ensure that treatment plans and goals for transition are completed and reviewed on a monthly basis. Provide feedback to participants. Make necessary referrals to outside agencies or departments as needed. Case manage community based-services. Complete regular check-ins with participants. Document participant interaction (i.e., client notes) as needed. Assign and track completion of Client Behavioral Interventions (CBI's) as needed. Create an after care/discharge plan for each assigned participant. Facilitate CBT manualized groups to fidelity. Submit to and conduct group audits. Utilize technology for programming needs (i.e., virtual meetings/groups or hybrid models). Keep participant data and information updated in the system (Client Information Management System or CIMS). Transport or conduct drug testing on participants upon staffing deficiencies. Participate in all meetings and trainings as requested by the supervisor(s). Perform other job responsibilities as directed by the supervisor(s).Education/Experience/Physical Requirements:
Bachelor's Degree from an accredited college or university. Two (2) years of supervised counseling experience or four (4) years of experience delivering manualized cognitive behavioral treatment. Work with high risk populations including substance abusers, court involved individuals, and trauma survivors. Preferred licensed by professional board of practice in counseling, mental health, social work, or alcohol and drug abuse. Good oral, written, and presentation skills. Strong computer skills including Microsoft Word, Excel, and Power Point. One early evening availability weekly. Some traveling required for meetings and trainings. Must have a valid driver's license in good standing. Must submit and pass drug testing upon hire. Must be able to sit, stand, bend and walk during the work day.
