Families are identified and referred to Catholic Charities of the Diocese of Fall River by DHCD (Department of Housing and Community Development). The main focus of the emergency assistance shelter program is to help families achieve successful placement into stable housing. Additionally, Catholic Charities connects families to resources that are available within their respective communities to sustain long-term stable housing. The Case Manager works with families on accessing additional housing resources, providing life-skills and financial empowerment training. Duties and Responsibilities The Case Manager is responsible for ensuring the functions of the program are met while providing services that are safe, timely, effective, efficient, equitable and client-centered. Ability to work as part of a team with a commitment to end homelessness Encourage and promote sustainability and self-sufficiency by proving enough assistance to help move to more stable housing Connect clients with community resources to meet their needs for employment, public assistance, education and other services Schedule Supportive Services for clients in and around neighboring communities related to mental health, clinical issues and substance abuse Must exhibit strong assessment skills, excellent documentation skills, organization, and communication skills to adequately handle a caseload and foster connections for services and community resources Must adhere to all DHCD program requirements Responsible for submitting a CORI (Criminal Offender Record Information) form and conducting a credit check on all new clients Responsible for the documentation, case files and maintenance of client information Meet with Program Supervisor for weekly supervision and updates on client's progress Meet with clients weekly to evaluate progress, identify potential barriers and develop strategies Respond to inquiries, phone calls, correspondence, and e-mails in a timely manner Coordinate inspections on empty shelter units with the maintenance department, followed by a walk through inspection prior to client entry Conduct shelter unit inspections by-weekly and report conditions to Program Supervisor Complete all tasks as assigned by the
