POSITION SUMMARY
The Case Manager works directly with families in the EA Family Shelter Program to identify strengths, assess needs, and address barriers to rehousing, overall well-being, and self-sufficiency. The Case Manager empowers families to drive their own case management planning and goal setting, in close collaboration with Housing Search staff, consistent with the FY27 Scope of Service.
ESSENTIAL DUTIES & RESPONSIBILITIES
Meet with families in person at a minimum weekly (Bridge Shelter Track) or twice weekly (Rapid Shelter Track); meetings should be family-centered and accommodate work, school, and childcare schedules. Conduct a full assessment of each family's individualized strengths and needs, coordinating access to resources to address unmet needs. Develop community connections for families in areas including financial literacy and budgeting, employment, food and nutrition, children's education and childcare, adult education/training, healthcare, cultural/spiritual connectedness, substance use support, trauma recovery, family relations, and domestic/family violence support. Review CORI records to help address barriers to rehousing, and support credit repair and eviction history mitigation efforts. Collaborate with Housing Search staff to develop and update Rehousing Assessments (RHAs) and Rehousing Plans (RHPs), and support families who are not complying with their plans. Assist families with resolving barriers to rehousing, such as securing identification documents, resolving outstanding warrants, or addressing rent/utility arrearages. Support referrals to DPH's Follow-Up Outreach Referral (FOR) Families program and other Intensive Case Management resources as appropriate. Support parents in addressing children's educational and health needs, including school, childcare, and early intervention enrollment. Connect adult household members to job training and employment services, and support applications for public benefits (SNAP, TAFDC, SSI/SSDI, etc.). Support families with EA Family Shelter program application completion, and with submission of shelter exit date extension requests (lease bridges, hardship waivers, Extraordinary Circumstances Waivers). Provide neighborhood orientation, including guidance on public transportation, food markets, health centers, schools, and places of worship. Document all case management efforts and meetings — including attempted contacts — timely and thoroughly as case notes in EOHLC's data management system.
QUALIFICATIONS
Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred; relevant experience may be considered in lieu of degree. 1-3 years of case management or social services experience, ideally with families experiencing homelessness or housing instability. Working knowledge of trauma-informed and person-centered care practices. Strong documentation and time-management skills. Bilingual candidates strongly encouraged to apply

Commitment to trauma-informed, person-centered, and culturally responsive practice with families experiencing homelessness. Ability to maintain professional boundaries and strict confidentiality of family and personnel records. Strong written and verbal communication skills; ability to communicate effectively with families, colleagues, and EOHLC staff. Ability to remain calm and exercise sound judgment in emergency or crisis situations. Bilingual/multilingual ability strongly preferred given the linguistic diversity of families served. Ability to build trusting relationships with families from diverse backgrounds and circumstances. PHYSICAL DEMANDS & WORK ENVIRONMENT
Position is based at a shelter site and/or program office; may require travel between sites. Ability to sit, stand, walk, and climb stairs for extended periods; ability to lift/carry up to 25 lbs (e.g., supplies, linens). Ability to respond promptly to emergencies, including evacuation procedures, which may require rapid movement. Must successfully pass a CORI and SORI background check prior to hire and periodically thereafter, consistent with EOHLC policy. Flexibility to work some evenings, weekends, and holidays as program needs require
