The Stabilization Specialist provides continued case management support to families after they exit shelter into stable housing through the HomeBASE program, helping ensure families maintain long-term housing stability and do not experience homelessness again, consistent with the FY27 Scope of Service.
ESSENTIAL DUTIES & RESPONSIBILITIES
Meet with families who have exited shelter and are receiving HomeBASE benefits at least once per month, in person, on a family-centered schedule. Provide ongoing support to help families maintain stable housing, including assistance with landlord challenges and referrals to community or state programs (e.g., childcare, community health resources). Maintain regular communication with landlords, property managers, or host families to proactively identify and mediate tenancy issues. Coordinate with the Regional Administering Agency (RAA) regarding HomeBASE adjustments, such as rental stipend/rent-share changes, and notify the RAA within two business days if a stipend must be discontinued. Conduct housing search activities if a family needs to relocate or is at risk of losing housing. Support families in accessing housing court resources or local legal aid if facing housing instability. Support families' access to homelessness prevention resources, including Residential Assistance for Families in Transition (RAFT) applications. Support EA Family Shelter program reapplication for families in Stabilization when a recurrence of homelessness is unavoidable despite reasonable efforts. Provide the full range of case management activities described for Case Managers (e.g., connections to benefits, employment, healthcare, education) as relevant to each family's stabilization needs. Ensure a seamless transfer of stabilization case management to the appropriate Home

when a family's location makes continued direct service impractical. Document all stabilization efforts and meetings — including attempted contacts — timely and thoroughly as case notes in EOHLC's data management system.
QUALIFICATIONS
Bachelor's degree in Social Work, Human Services, or related field preferred; relevant experience may be considered in lieu of degree. 1-3 years of case management experience, ideally supporting housing stability or homelessness prevention. Working knowledge of the HomeBASE program and homelessness prevention resources such as RAFT. Strong communication and mediation skills for working with landlords and community partners. Valid driver's license may be required for community-based visits

Commitment to trauma-informed, person-centered, and culturally responsive practice with families experiencing homelessness. Ability to maintain professional boundaries and strict confidentiality of family and personnel records. Strong written and verbal communication skills; ability to communicate effectively with families, colleagues, and EOHLC staff. Ability to remain calm and exercise sound judgment in emergency or crisis situations. Commitment to non-discriminatory practice consistent with the Fair Housing Act and Americans with Disabilities Act (ADA). Bilingual/multilingual ability strongly preferred given the linguistic diversity of families served. Ability to work independently in the community with strong self-direction and time management. Skilled at proactive relationship management with landlords to prevent housing loss.
PHYSICAL DEMANDS & WORK ENVIRONMENT
Position is based at a shelter site and/or program office; may require travel between sites. Ability to sit, stand, walk, and climb stairs for extended periods; ability to lift/carry up to 25 lbs (e.g., supplies, linens). Ability to respond promptly to emergencies, including evacuation procedures, which may require rapid movement. Must successfully pass a CORI and SORI background check prior to hire and periodically thereafter, consistent with EOHLC policy. Flexibility to work some evenings, weekends, and holidays as program needs require. Frequent local travel to meet families and landlords in the community; reliable transportation required.
