Job Description

Prestige Adult Foster Care has been operating since 2017 and is committed to providing quality care to individuals throughout Massachusetts. If you are a Case manager seeking a flexible position with the opportunity to make a positive impact on individuals and families, we encourage you to apply.
KEY ACTIVITIES
This is position allows you to schedule your clients according to what works best for you. Your caseload will consist of clients who require monthly or bimonthly home visits. The number of clients on your caseload will depend on the number of hours you wish to work. Regular home visits
  • Conduct home visits with each individual at the qualified setting bi-monthly (alternating with the bi-monthly visit by the registered nurse) for Level I, and monthly Level II.
Provide counseling and support for the family, monitor health and safety in the home, and regularly receive and review the AFC caregiver log. Support and train families/caregivers in conjunction with the RN Provide referrals to community service organizations when needed
  • Assist individuals and families to advocate for their needs, providing information and referral to community resources when required Provide case management as needs arise
  • Assist families with coordinating care and services. Document all visits and communication and complete required MassHealth documentation
  • Complete a care management progress note corresponding with each on-site visit and upon significant change.
OTHER DUTIES AS ASSIGNED, INCLUDING BUT NOT LIMITED TO

Coordinate on-going assessments of the home, monitoring for conditions that affect the health and safety of the individual Review and document the first and safety procedures for the home Coordinate visits and support with AFC Nurses Assist families with the MassHealth documentation requirements Perform other tasks, as needed or assigned.
Essential Physical Requirements:
The ability to move independently throughout a wide range of environments, some of which may not be wheelchair or otherwise handicapped accessible, is required. Independent transportation to locations that may not be served by public transportation may be necessary. Prestige AFC will make reasonable accommodations, whenever possible, when needed and/or requested by employees. Bachelor's Degree in Psychology, Sociology or related field Two (2) years' experience in the care of elderly or persons with disabilities Experience in working with families in their homes, preferred Must have valid driver's license and/or reliable transportation
