Job Description

Classification:
Hourly/Non-Exempt Department:
In-Home Supports Reports to: Program Director Position Overview The Community Based Case Manager is responsible for: Developing and implementing Individual Service Plans (ISP) . Supporting community inclusion and independent living skills . Providing care coordination , direct care , and supervision as needed—including personal care and daily living activities . This role aligns with the Charles River Center's values of self-direction and follows our established principles, policies, and procedures .
PRIMARY DUTIES
Coordinate care and provide support in: Education and social skills Meal planning and grocery shopping Personal care and hygiene Clothing care and travel training Money management and housing Community participation and religious practices (if desired) Transport individuals to medical appointments and: Follow up on medical care plans Educate individuals, families, and staff on self-directed care Support healthy, culturally appropriate meal planning and purchasing Provide transportation and skill-building in grocery shopping

Adheres to agency policies and guidelines in all professional interactions.

Maintains reliable attendance, arriving on time for assignments, meetings, and training.
Initiative:
Demonstrates self-reliance in problem-solving and carrying out job responsibilities.
Management:
Supports and enforces agency policies while leading by example.
Data Collection:
Gathers and shares relevant data as indicated by the family or CRC Support Navigator . Bachelor's degree (or currently enrolled) in Human Services or a related field Equivalent experience may be considered in some cases
Preferred:
Experience working with individuals with developmental disabilities Familiarity with behavior modification techniques Strong oral and written communication skills Valid Massachusetts Driver's License and reliable personal transportation Proficient in Microsoft Office and general computer use
LANGUAGE SKILLS
Fluent in English with the ability to read and interpret documents. Ability to communicate effectively with clients, colleagues, families, and community members . Strong written and verbal communication skills for reports, correspondence, and presentations .
PHYSICAL DEMANDS
Employee must have significant endurance to sustain the physical demands of the role. Regular activities include sitting, standing, walking, stooping, kneeling, or crouching . Frequent use of hands/fingers to handle objects and tools. Ability to generate force to maneuver up to 100 pounds (assistance provided when possible). Must be able to lift up to 30 pounds from floor to waist and 10-15 pounds from waist to overhead .
REASONING ABILITY
Ability to follow detailed written and oral instructions .
