  • Full-time What You'll Do Build trusting relationships with clients and families, grounded in respect and person-centered care Conduct initial and ongoing assessments to understand strengths, needs, and goals Develop and coordinate individualized service plans that promote independence and well-being Connect individuals to community resources, healthcare services, and supports Collaborate closely with interdisciplinary teams, providers, and community partners Conduct home visits and facilitate care planning conversations Maintain consistent communication and complete timely, accurate documentation What You Bring A commitment to serving older adults and individuals with disabilities with dignity and compassion Strong organizational skills and the ability to manage multiple priorities in a dynamic environment The ability to work independently while remaining deeply collaborative Excellent communication and relationship-building skills across diverse communities Qualifications Bachelor's degree preferred Minimum of two years of professional experience working with individuals age 65+ At least one year of experience in a healthcare setting serving older adults (e.
