Our Transitional Assistance team works closely with individuals, case managers, healthcare and rehabilitation facilities, housing providers, community organizations, and other partners to help individuals overcome barriers to successful community transitions. We are seeking a compassionate, resourceful, and highly organized Transitional Assistance Coordinator to join our Woburn team . What You'll Do As a Transitional Assistance Coordinator, you will manage a caseload of individuals and coordinate the services, documentation, housing activities, and resources necessary to support successful transitions into the community.
Your responsibilities will include:
Conducting initial meetings with individuals to identify their needs, goals, preferences, and barriers to transitioning into the community. Developing, completing, submitting, and updating individualized Transitional Assistance Plans. Working with individuals and their case managers to identify services, expenses, and supports needed for a successful transition. Completing housing applications with individuals and assisting with required housing documentation. Searching for appropriate housing opportunities and helping individuals navigate the housing application process. Following up with housing authorities, property managers, landlords, and housing organizations regarding applications and housing opportunities. Assisting individuals with obtaining identification, birth certificates, Social Security documentation, income verification, and other required documents. Coordinating move-in needs, including approved furniture, household essentials, utilities, moving services, transportation, and other transition-related supports. Conducting home, facility, and community visits throughout Greater Boston, Middlesex County, and surrounding communities. Scheduling and accompanying individuals to housing, Social Security, RMV, banking, and other transition-related appointments when appropriate. Coordinating with case managers, healthcare and rehabilitation facilities, housing providers, landlords, vendors, families, and community organizations. Obtaining required quotes, receipts, invoices, approvals, and supporting documentation. Tracking approved expenses and ensuring purchases and services remain within each individual's approved Transitional Assistance Plan. Maintaining detailed and timely case notes for client contacts, activities, appointments, and services provided. Maintaining complete, accurate, organized, and audit-ready client records. Monitoring each individual's transition progress and following up on outstanding tasks, applications, documents, and approvals. Providing regular and timely updates to case managers and other members of the individual's support team. Identifying barriers that could delay a transition and taking initiative to help resolve them. Ensuring services and documentation comply with Care Remedy policies and applicable Transitional Assistance program requirements. Providing respectful, person-centered, compassionate, and professional support throughout the transition process. What We're Looking For Bachelor's degree in social work, human services, healthcare administration, psychology, sociology, nursing, or a related field preferred. Experience in case management, human services, housing, healthcare, senior services, disability services, or community-based programs preferred. Experience completing housing applications or assisting individuals with housing resources is highly desirable. Excellent organizational and time-management skills. Strong written and verbal communication skills. Exceptional attention to detail and documentation. Ability to manage a caseload with multiple deadlines, appointments, applications, and priorities. Strong problem-solving skills and consistent follow-through. Ability to work independently while collaborating with a multidisciplinary team. Comfortable working in the community and visiting individuals in homes, rehabilitation facilities, hospitals, and other settings. Comfortable using electronic case-management systems and technology.
