Cornerstone Montgomery - Case Manager of Assertive Community Treatment (ACT) Cornerstone empowers people living with mental health and co-occurring substance use disorders to live, work, and integrate successfully into the community. Responsibilities of the ACT Case Manager Assist with Housing and benefits needs of ACT clients such as: HIP reports, benefit applications and renewals, HIMS tracking, monthly budgets, rep payee and other tasks to support client's independence and meeting basic needs. Coordinate SOAR services to support ACT clients with applying for disability income. Provided case management support by connecting clients with appropriate entitlement and social service resources. Helping client access and complete benefit eligibility and the application processes. Provide supportive counseling and assertive case management to include: appropriate assessment, planning, coordination, linkage, delivery and advocacy of services for all assigned clients Qualifications of the ACT Case Manager Bachelor's degree and one (1) year of case management experience required. Equivalent combination of education and experience may be substituted Experience working with the population of serious mental illness in the capacity of case management, integrated care coordination, and medical office organization strongly preferred Must have access to a personal vehicle and be willing to use it for work purposes.
