Job Description

Q-Care is seeking an experienced and compassionate Case Manager to support individuals with developmental and co-occurring disabilities. The Case Manager is responsible for person-centered planning , service coordination , regulatory compliance , family communication , and quality assurance oversight to ensure individuals receive the highest quality support and achieve their personal goals. Responsibilities Develop, implement, and monitor Person-Centered Plans (PCPs) , including DSATs and SIPs . Conduct assessments and coordinate services to meet the unique needs of individuals receiving support. Ensure compliance with
DDA , LTSS , COMAR
regulations, and agency policies. Monitor goal progress and review documentation and data submitted by direct support staff. Maintain ongoing communication with individuals, families, guardians, and interdisciplinary teams. Collaborate with Quality Assurance staff regarding incident reporting, investigations, and follow-up actions. Advocate for individuals and promote community inclusion, independence, and personal growth. Build relationships with community partners, service providers, and regulatory agencies. Assist with crisis intervention, problem-solving, and service planning as needed. Qualifications Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation Services , or a related field. Minimum of 1 year of program management experience or 3 years of experience working with individuals with developmental disabilities. Knowledge of Maryland DDA, LTSS, Person-Centered Planning, and COMAR regulations . Strong case management, organizational, documentation, and communication skills. Proficiency in Microsoft Office Suite. Experience with Therap preferred. Ability to manage multiple priorities while maintaining attention to detail.
