Vocational Case Manager Position Type Full Time Description $17.75 Hourly Rate
Job Summary:
The Vocational Case Manager is responsible for client recruitment, case management, facilitation of employment and career plans and provision of on-the-job and off-site support to assigned clients seeking or in community-based employment.
Essential Functions:
Participate in the development and implementation of case finding and client recruitment initiatives. Act as case manager for assigned clients and provide information about and referral to services needed through community resources (i.e.: adult education classes, childcare, transportation, credit counseling). Implement training and interventions to further career development of assigned clients, including job readiness training, job seeking support, employment retention support, and barrier resolution. Act as liaison between assigned clients, employers and referral and funding sources. Provide post-employment support and assist clients to identify and utilize natural support systems. Educate clients, community organizations and employers concerning HGI programs and services. Act as an advocate for assigned clients. Cooperate and maintain good rapport with clients, employers, staff and referring agencies. Maintain all relevant client records, case notes and other documentation including attendance records and progress reports. Complete and submit quarterly reports and program proposals to funding source(s) as directed. Complete monthly progress reports and submit fee-for-service invoices as directed. Assist with program evaluation and community needs assessment to identify program and service needs. Participate in development and submission of program and service proposals to expand and diversify program funding as directed. Participate in agency meetings and committees as directed. Qualifications
Education and Experience:
Associates degree or high school diploma and two years of relevant experience required, Bachelors degree preferred. Personal experience with public assistance system preferred. Must possess clean driving record and provide own transportation.
