Job Description
Cornerstone -In-Home Intervention Program (IHIP) Rehabilitation Counselor Cornerstone empowers people living with mental health and co-occurring substance use disorders to live, work, and integrate successfully into the community. We envision that each person we serve will have a meaningful life of their choosing. We offer years of evidence-based treatment experience to our consumers and practical training to our staff. Our robust training program allows staff members to develop and grow in the position and provides the basis for upward mobility within the agency. The In-Home Intervention Program (IHIP) provides intensive in-home services to children, adolescents and their families to increase age-appropriate skills and resiliency. Responsibilities of the IHIP Rehabilitation Counselor Provide high quality psychiatric rehabilitation & case management to children, adolescents and their families Work with the child's family in the home environment to increase skills to prevent any future out-of-home placements. Attend Individuals Education Plan meetings (IEP meetings) as required to serve as an advocate for the child's needs in the school system. Develops Treatment Plans and identifies the child's barriers to community integration and develops strategies to offset them in consultation with the child and their family. The IHIP Case Specialist will train with a senior IHIP employee for a minimum of 60 hours prior to their first encounter with child/adolescent independently as per regulations. Qualifications of the IHIP Rehabilitation Counselor Bachelor's Degree in Social Work, Psychology, Rehabilitation Counseling, or a related discipline is required Driver's license, proof of auto insurance, and access to a vehicle for work purposes Knowledge of basic computer functions (Google Suite, Microsoft Office) and ability to learn software systems and/or websites (Paycom, EHR, Relias, etc) Additional experience must consist of at least one (1) year in direct service Our company offers a dynamic and collegial work environment, ongoing opportunities for professional growth, and generous benefits, including 3 weeks paid vacation, up to 500 hours of sick leave, 9 paid holidays and 2 floating holidays (bonus days). Eligible for health care (single and family) the 1st of the month following 30 days of employment.
