Job Description
Fulltime position coordinating services for persons with disabilities. Set up and attend Person Centered Plan team meetings as required. Frequently checks with individuals in their residential and day settings to ensure their needs are being met. Be the point of connection between individuals and their natural supports. This position requires either a Bachelor's Degree in Human Services or a Bachelor's Degree and prior experience. The main hours for this position are M-F 8am - 4 pm. Additional hours are dependent on the needs of the organization. This position has a full benefit package. Must be able to pass a drug screening, background check and DOT physical.
