We are a gold standard non-profit that serves individuals with disabilities. We are looking for highly qualified candidates who would like to work in a family-friendly environment.

Experience and Qualifications:

• Bachelor's degree in social work, sociology, psychology, counseling, education, or any related human services field.
• Must have a current driver's license and reliable vehicle to perform required responsibilities.
• Must successfully complete a criminal background check.

Responsibilities include case management services for individuals with disabilities and seniors to achieve their independent living goals through the planning of community-based support funded by Medicaid. Courteous phone etiquette, excellent written and verbal communications skills, computer skills - literate in Microsoft Word. Preference will be given to those who have experience in providing case management to persons receiving long-term care and/or home and community-based services, working knowledge of Medicaid regulations, and Medicaid waiver.
