Summary:
The ATCL Case Manager provides essential support to patients and community members by responding to referrals from MaineHealth providers and managing incoming calls from individuals seeking information about Social Determinants of Health (SDOH) resources. This role includes conducting timely outreach to patients, assessing needs, and guiding individuals to appropriate services that improve health and wellbeing. Case Managers also screen patients for eligibility across bothAccess to Care (ATC) internal programsandexternal community programs, ensuring that patients are connected to the most suitable supports. This includes helping patients navigate healthcare access, financial assistance, community resources, and other critical SDOH services.

Working closely with healthcare providers, social service organizations, and the broader ATCL team, the Case Manager ensures accurate documentation, highquality customer service, and seamless care coordination. This is a fulltime position, Monday through Friday, 8:00 a.m. to 4:30 p.m. Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education:
Bachelor's Degree or two years relevant experience preferred.
License/Certifications:
Certification as a Maine Enrollment Assistor or Certified Application Assistor with the Health Insurance Marketplace preferred or will be trained upon hire. Valid Driver's license required.
Experience:
Two years of relevant experience in social/human services, health care or related field preferred. Knowledge of state and federal agencies, community resources, experience with community outreach and networking, and strong advocacy skills preferred.

Thebase rangeis an estimate and does notreflect the full valueofourtotal compensationpackage. The pay range for this position is $52,499.20to $71,052.80per year. Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
