Summary of Responsibilities The Supports Coordinator Assistant provides case management services under the immediate supervision of a Qualified Mental Health Professional (QMHP), Qualified Intellectual/Developmental Disabilities Professional (QIDP), or Child Mental Health Professional (CMHP). Responsibilities include assessment, service planning, advocacy, monitoring, linking, and coordinating services. The SCA supports implementation of the Individual Plan of Service (IPOS) by promoting independence, recovery, personal goals, and community inclusion. Additional duties include assisting individuals in accessing financial, medical, and social supports; coordinating care with health providers; and offering referrals to community resources. This role may assist with IPOS development and monitoring, oversee out‑of‑catchment placements (COFR), and serve as a member of the Children's Diagnostic and Treatment Team, including completion of required annual child‑specific continuing education (24 CE hours per Policy 5135). Essential Job Functions Maintain effective case documentation in accordance with Person‑Centered Planning and NeMCMHA Case Recording Guidelines. Collaborate with staff to assess needs and develop person‑centered, family‑focused treatment plans. Link, monitor, and coordinate authorized services outlined in the IPOS. Assist individuals with applications for financial, housing, medical, and entitlement benefits. Consult with agencies, schools, and community organizations as needed. Participate as an active member of the Children's Diagnostic and Treatment Team. Demonstrate knowledge of universal precautions, confidentiality standards, recipient rights, and emergency protocols. Non‑Essential Job Functions Perform other duties as assigned. Participate in the Board's continuous quality improvement initiatives as requested. Provide own transportation for agency‑related responsibilities when required. Complete all required training. Physical Requirements Must be able to walk, bend, stand, sit, lift up to 25 pounds (with or without assistance), reach, hear, see, perform hand/finger movements, and drive.
