Job Description

If so, we have an exciting opportunity for you to work as afull time (40hrs/wk) Case Manager! What You Will Be Doing As a Case Manager on our Community Treatment Team (CTT) , you will provide linking, advocacy, coordinating and monitoring to adults with a serious mental illness and/or substance use disorders that are members of our case management program. You will provide culturally responsive services and will work closely with multiple public service systems, including ISK, Kalamazoo Court System, DHHS and the hospitals. Responsibilities also include providing outreach services, evaluating individuals for safety, and completing clinical documentation accurately and in a timely manner. This position primarily supports specialized residential case management services and requires travel both within and outside Kalamazoo County. While annual documentation is not a primary responsibility, the successful candidate must maintain a strong understanding of these requirements and be proficient in completing related documentation as needed. What We Require You will have a minimum of one year of experience working with adults with serious mental illness, substance abuse and/or co-occurring disorders that also have co-morbid health conditions. You will have a Bachelor's degree in Psychology, Social Work, or a related field. A valid Michigan Motor Vehicle Operator's license and reliable transportation are required. Working Conditions:
Must be able to perform essential job functions with or without reasonable accommodations, including, but not limited to, visual and/or audiological appliances and devices to increase mobility. Physical Efforts - Job demands include prolonged sitting and standing. Requires light lifting up to 25 pounds, stooping, kneeling, crouching, or bending. Requires coordination of hands and/or eye/hand/foot and ability to hear and communicate verbally Working Conditions - In office environment with noise from computers, copy machine, and telephones. Use of computer for periods in excess of 30 minutes at a time. May be exposed to bloodborne pathogens, infectious diseases, and parasites. May encounter community environments that include homes, schools, courts, and other community settings that may or may not be barrier-free. Travel throughout the Kalamazoo area may be required.
