Job Description

Bachelor Level Social Worker/Case Manager - Montcalm Care Network MCN is seeking a Case Manager. Montcalm Care Network serves adults and children with severe mental illness and intellectual developmental disabilities while specializing in models of integrated health and co-occurring treatment. MCN is the respected leader of behavioral health care in our community and actively partners other community providers. Competitive pay and benefits with average pay for this position ranges from $51,894 to $73,980 depending on experience. Case Managers provide community-based outreach to persons with mental illness and are responsible for linking, coordinating, monitoring and psychoeducation. Our case management model utilizes a team approach with nurses, peer supports, and paraprofessional staff providing comprehensive care. Position will provide services to populations of adults with severe mental illness or intellectual developmental disability. This position may provide case management services for individuals with intellectual development disability (IDD) and severe mental illness (SMI) primarily residing in an Adult Foster Care home. Student Loan Repayment:
MCN is an approved National Health Service site (NHSC, a loan repayment program); in addition to Public Service Loan Forgiveness (PSLF), and MDHHS Behavioral Health Loan Repayment Program (BHLRP). MCN offers tuition reimbursement of up to $5,250 per year.
Employment Qualifications:
Education:
At minimum possession of a bachelor's degree in social work or a related human services field
Experience:
One year of professional experience working with persons with intellectual/developmental disabilities or severe mental illness
Other Requirements:
Licensure or limited licensure as a Social Worker or must be a qualified CMHP and/or QIDP or supervised by individual with this credential. Possession of a valid State of Michigan driver's license and ability to obtain and maintain a chauffeur's license within 30 days of hire. Local travel is an essential function of this role.
