This position requires a compassionate, resourceful, and highly organized individual who can effectively collaborate with a team to help empower clients to achieve stability and independence.Responsibilities:
Coordinate, make referrals, and follow up for services and resources as needed. Complete all admission and discharge paperwork in a timely manner. Attend and participate in case consultations, and supervision meetings. Assist clients with securing housing, and interface with landlords to maintain housing. Provide support services once housed to help improve stability and longevity in housing. Use motivational interviewing techniques and engagement strategies to build an alliance with clients. Requirements & Qualifications Bachelor's degree in Social Work, Psychology or a related human services field. 1 year of experience providing services to individuals with a mental illness or substance use disorder required. Experience working with housing clients preferred. Must qualify as a Mental Health Practitioner in the State of MN. Licensed as a Social Worker in the State of MN preferred. Provisional License in the State of MN will be considered. Valid driver's license and proof of auto insurance. Why Zumbro Valley Health Center? $25.11 - $30.13 per hour. Compensation is based on years of experience.
