Job Description
Under the supervision of the unit supervisor, the Case Management Associate performs supportive and adjunctive case management and community support services. This position provides services to adults experiencing serious and persistent mental illness within the Carver County Community Support Program (CSP). This position may be safety-sensitive, depending on assigned duties and responsibilities.
- Ability to do and document CSP activity groups.
- Perform outreach to potential clients including assistance in completing forms, providing assistance in understanding the nature of available services and how potential clients may qualify for and benefit from these services.
- Assist case manager in work with clients on a variety of housing supports, including tenancy rights, working through conflicts with neighbors and/or landlords, and assist clients in keeping current with rent payments.
- At direction of case managers or case management supervisor, lead a variety of supportive and rehabilitative groups to adults with serious and persistent mental illness.
- Provide 1:1 support to assist adults in accessing services and resources, including provision of transportation.
- Other duties as assigned.
- High school diploma or GED
- Requires valid MN driver's license or evidence of equivalent mobility depending upon program or assignment.
Preferred Qualification:
- One year experience working directly with clients in a human services/community services area and setting.
