Lees Summit, MO and Overland Park, KS Hours:
M-F 8:30a-5:00p This position is responsible for coordinating all aspects of the Hospice Bereavement Program offered to grieving families. These services are offered in a collaborative and interdisciplinary manner. The Bereavement Supervisor does not manage employees but supervises the provision of bereavement programming and will be responsible for but not limited to: 1. Develops and coordinates bereavement plan of care based on psychosocial and bereavement assessments 2. Completes comprehensive grief risk assessment, assessing for the physical, emotional, financial, cognitive and social needs of bereaved clients 3. Maintains and completes all required records, documentation and reports to ensure compliance with all state and federal regulatory requirements 4. Attends and participates in interdisciplinary group meetings 5. Provides pre-death bereavement support to patients and families 6. Provides bereaved clients with grief literature, notes, and bereavement letters during bereavement period of 13 months post-death 7. Offers in person grief support to bereaved clients and provides appropriate referrals for ongoing counseling/community support 8. Coordinates and/or facilitates community-wide bereavement support groups. Promotes bereavement support groups and other bereavement support services to the community 9. Promotes the continued development and utilization of bereavement literature including updated list of community bereavement support groups, individual counseling services, adult and children's grief support resources 10. Assist in supervision and support to bereavement program volunteers 11. Organizes and facilitates annual Celebration of Life/Memorial service 12. Accepts and performs other related duties and responsibilities as required 13. Provides administrative support to the Volunteer Services department, including assistance with volunteer documentation, communication, scheduling coordination, and other departmental support tasks as assigned 14. Collaborates with Volunteer Supervisors to support volunteer program operations and ensure timely completion of required administrative processesRequirements :
Bachelor's or Master's degree in a related field such as social work/pastoral care/counseling. Clinical experience or skills relevant to grief and loss; Excellent listening, assessment, critical thinking and problem solving skills.
