Job Description
The Care Service has an immediate need for a full-time (40 hours a week) Case Manager to serve the basic needs of low income unhoused individuals and families in crisis in St. Charles , Lincoln and Warren Counties in MO. Responsibilities include utilizing assessment tools to focus on basic needs, helping stabilize those in immediate crisis, establishing a long term plan for recovery and locating housing. Other duties include (but are not limited to) working with individuals, families and children in search of basic need items including but not limited to: providing emergency food or enrollment at local food pantry; a referral for clothing or furniture, school supplies, or any connection to mainstream resources that will benefit the stability of the family. This candidate must have a Bachelor's Degree in Social Work or a like field. Preferred 1+ years' experience with intensive case management, interviewing methods, case planning and documentation experience. We will consider recent college graduates with practicum experience. Candidate must have effective communication skills, verbally and in writing. Candidate must have reliable transportation for home visits (mileage reimbursement available). FYI- After employees have passed their 90-day probationary period employees are able to go to a 4 day- 10-hour workday. This is not mandatory.
