Job Description

  • This position is only available while The Kitchen, Inc.

As a Parenting Youth Case Manager, you will serve as one of our Frontline Heroes at Rare Breed. Rare Breed is a central hub that provides immediate access to basic needs and rapid access to community services for homeless and at-risk youth ages 13-24. Personal Time Sick Leave Vacation Time Paid Holidays Major Areas of Responsibility:
  • Lead quarterly groups with parenting and pregnant youth to formally discuss ways the program could improve or expand its services.
  • Maintain accurate documentation of parenting and pregnant youth interactions and participation.
o How many pregnant and parenting youth the case manager serves each year, including the number of children. o Conduct surveys to determine what services the pregnant and parenting youth would like to receive. o Have annual follow-up surveys and results showing how effective the case manager has been in connecting the youth and their children with services and helping to improve their situations (surveys need to be completed before May 1, so the results can go in the annual report) o Have quarterly "check-in" groups to formally discuss ways the program could improve or expand its services
  • Educational Emphasis on Early Child Development
  • Maintain the safety of youth and staff by remaining alert to concerning behaviors in Rare Breed and addressing behaviors before they escalate.
  • Other duties as assigned.
Job Requirements:
Education and Experience
  • Bachelor's degree preferred or Associate's degree required, preferably in the Human Service or Early Child Development field
  • One to two years' experience in the Social Service field or experience that can be related to providing Case Management.
  • Experience working with parenting and expecting youth and/or early child development.
  • Experience working with individuals who have mental health and/or substance use disorders is a plus.
  • Working knowledge of Housing First Philosophy, Trauma Informed care, Motivational Interviewing, Critical Time Intervention, Progressive Engagement a plus.
  • Experience utilizing de-escalation techniques preferred.
